Incident Description
2nd stage reg hose separated form fitting where it attaches to the 1st stage rendering the reg useless. Fortunately the incident happened within 20m of surface and a safe return was made on spare cylinder.
Lessons Learned
Pay more care to the state of LP hoses and replace more frequently.
