Incident Description
An experienced cave diver was exploring and surveying a system in Florida and by his own admission had been pushing his air margins beyond the thrids rule. On return from exploration and surveying the diver ran out of air and died. It is possible that line entanglement was a contributory factor.
Lessons Learned
Gas management is critical.
