Incident Description
1965 - A diver in this cave which has short restricted sumps twice knocked his first stage valve partially off his pillar valve causing O-ring failure. He was able to get out quickly. Personal communication has revealed at least three other cases of O-ring failure on the A-clamp adaptor during the 1970s.
Lessons Learned
Since the 1980s the use of DIN fit valves is common place to avoid this issue - Ed.
