Leaking low pressure hose

Incident Description

Some 750 metres into the cave the diver noticed a significant stream of bubbles coming from the connection between the first stage and second stage hose on a stage bottle. The dive was turned and an uneventful exit made. Prior to the dive some tweaks had been required to the regulator and the connection had not be made firmly enough. In addition to this the stage had not been removed to navigate thegrabby entrance series to the dive and it is possible that the stage had been banged.
