High pressure hose failure

Incident Description

High pressure hose failed suddenly and catastrophically, when turning on valves prior to entering water. The hose was a 15cm Beaver branded hose, that had been bought new after 2017, and as outwardly in good condition. The failure occurred at the first stage end; the hose parted company with the metal fitting. The valve was turned off and the parts recovered. A spare hose was borrowed for the dive.

Lessons Learned

Hose failure is possible, notably when pressurising valves. Opening valves cautiously and at arms length may be prudent. Access to a spare hose saved the dive. Inside a cave, this could have caused ear damage and had implications for safe exit. Leaving hoses pressurised may lessen the risk. A blanking plug, or mini-gauge, may be fitted if parts and tools were carried. Buy quality equipment and do not bend hoses unduly.
