Cylinder bungee failure
Incident Date
2024-11-11
Incident Description
The left hand cylinder dropped out of its bungee a short way into the dive. The bungee remained intact and clipped in. The cylinder was reattached successfully. Aborting the dive was considered, but the decision was made to continue the dive.
Lessons Learned
The equipment (12L steel cylinder, Apeks DS4 valve, XDEEP wing) was as used previously on numerous dives, and the method of attaching the cylinder was conventional. The failure mode is not known.
Choosing a location / date with good visibility allowed an unforeseen issue to be identified and rectified easily. Diving as a pair meant assistance was available (though not needed on this occasion).
