Incident Description
On surveying dive at 250 m one of the divers regulators sprang a leak from the second stage swivel. Neither could be fixed so the diver limped back to base breathing from the leaking regulator whilst keeping the other fully functional set in reserve. The broken regulator whilst breathable was tending towards free-flow due to the leak therefore the cylinder valve had to be opened and closed by hand to minimise this. To add insult to injury the divers HID lamp packed up. Ample air reserves were carried and the return dive could have safely been made on the other set.
