Another roller buckle failure

Incident Description
The diver was on his way back to base when the belt of hisBritish Sidemount harness became undone in constricted conditions. He re-threaded the buckle only to find that hed done it up around the line. The buckle was undone and refastened correctly.
Lessons Learned
Prevent such buckles becoming undone underwater by looping excess webbing back through the buckle or fitting a quick release buckle (as used on weight belts) on the tail leading from the buckle. This can also serve as a weight retainer. It is also important to be able to undo and redo ones harness underwater as the diver has on more than one occasion needed to perform this action.
